Direct Primary Care allows us to provide truly personalized care to our patients. Taking insurance out of routine health care eliminates waste in the medical system, and allows us to focus on you and your health. It’s a formula for great care.
A low monthly fee is paid directly to Meditique.
You get comprehensive health care at our office with virtually no extra costs, and easy access to your doctor through email, virtual visits, and same day or next day appointments.
Medication, lab testing, and other diagnostic tests are suddenly less expensive as we pass our at-cost pricing directly onto you.
Use your insurance or catastrophic coverage only when it makes sense for you, saving a ton of money.
Extended appointment times. With extra time, we can focus on managing health through lifestyle and individualized care, reaching for medications when necessary instead of as a first line of treatment.

Nourish Program (3 months): $2,500
Food is Medicine. There is no secret that a plant-based diet has been proven to not only prevent, but also reverse chronic diseases. Our bodies’ red blood cells carry sugar molecules around for 90 days. Any lifestyle changes you make should be sustained for at least 90 days (3 months) before you can measure any significant change. Dr. Mizelle does an initial home visit to assess your kitchen and pantry. She will provide a guided healthy grocery shopping tour and teach you tips for nutrition success. You will receive a Meditique Nourish Starter Kit and guidance on beginning your kitchen pantry makeover. Biweekly meal suggestions and tutorials will be emailed to you with a final follow up visit during month 3. You may get labs basic labs checked at the beginning at end of this program.
Healthy Home & Detox Program: (3 months): $2,500
Medicine goes beyond just treating an illness. Have you considered the health of your home and its affect on your well-being? Dr. Mizelle does an initial home visit to assess your sleep hygiene, toxin exposure, nutrition status, and stress indicators and anything that could be hindering your wellness journey. She provides detailed feedback and gives you a guide to improve your home’s health. there will be a 2 months check-in and a follow up visit during month 3.

We want to take care of all of your medical needs at our practice, however there are times when it is not medically appropriate to perform a service or procedure listed above in the clinic. In these cases where treating a health issue is beyond the scope of care of family medicine, a referral will be made to the appropriate provider. Like all care received outside of our office, this is not included in membership. Some examples of this situation are a fracture that is too complicated to cast, a laceration that is too severe or is in an area where a plastic surgeon is needed to prevent scarring, a biopsy that is too invasive or complex to perform in clinic, or a complicating health concern that makes a procedure unsafe. These decisions will always be made in the best interest of our patient's health, and will be at the discretion of the doctor.
